The US had very little in the way of Civil Defense (shelters, etc.) whereas the USSR made some significant efforts. However, neither side would have fared well as simply surviving the initial blast is only part of the issue. The resulting radiation levels would have been unlivable and lasted for generations. Little credit is given the technological advances that improved warhead accuracy to the point where smaller yield (150 Kilotons or less) warheads precisely placed would deliver superior results to the megaton warheads of old. Therefore a single MIRV missile could target and destroy 8 to 10 missile silos in a preemptive strike without targeting population centers. While this made (limited) nuclear war at once more defensible as a strategy it also frightened both sides into contemplating that nuclear was now more likely. This is (was) the motivating factor towards disarmament.
